Will rusty callipers affect how my brakes work?

Surface rust on the outer body of a calliper is a cosmetic problem. Painting doesn't touch the pads or pistons, which are masked off, and if you're worried about how the brakes feel, have a mechanic check them.

Do I need a photo of every calliper?

Send one of each calliper you want painted, so the quote covers the whole job. If you only want the fronts done, photos of those two are enough.

Can you judge from the photo how much work the callipers need?

Usually we can. The photo shows how much rust treatment a calliper needs and how the old finish is holding up, and the reply sets out the price and the work.
